Loan Document Electronic Delivery Agreement

This agreement describes your rights and obligations as a user of our electronic delivery service. It also describes the rights and obligations of Queensborough National Bank & Trust Company. Please read this agreement carefully.

By accepting the "Loan Document Electronic Delivery Agreement", referred to as "Agreement" below, you authorize us to make loan disclosures and notices, which we are required to provide to you under applicable Federal and State statutes and their implementing regulations, available to you electronically. Such disclosures and notices may include initial account/application disclosures, real estate appraisals and evaluations, late payment notices, rate change notices, and escrow notices.

The terms "you" and "your" refer to each person who consents to receive loan disclosures and notices in the form of an electronic communication. The terms "we," "us," and "our," refer to Queensborough National Bank & Trust Company (QNBT).

Electronic Delivery Requirements
To receive and retain your QNBT loan disclosures and notices, you’ll need a personal computer with a web browser such as Microsoft Edge, Google Chrome, Safari or Firefox. For the optimum user experience we recommend the use of the most recent available version within the last year. To view the disclosures and notices, you will need Adobe Acrobat Reader. This product is available for free at http://www.adobe.com . All other software, hardware and systems must be provided at your cost.

QNBT will deliver loan disclosures and notices to your current email address as provided and confirmed by you at the end of this agreement. You will receive loan disclosures and notices as an attachment to an email from us to you. You are responsible for accessing, opening and reading your disclosures and notices. These documents contain important and legally binding information and conditions. To retain a copy of these documents, your system must have the ability to either download or print PDF documents.

Documents delivered pursuant to this Agreement will constitute your only notice of matters contained in the documents. You are responsible for promptly notifying us if any documents you receive are not accessible or are incomplete or unreadable.

You may request a paper copy of any document delivered electronically at no cost by calling our Customer Care Center at 478-625-2000 or write to us at Queensborough National Bank & Trust Company, Loan Processing Department, PO Box 467, Louisville, GA 30434. Please allow ten (10) business days to process your request. Requesting a paper copy will not affect your participation in the electronic delivery of documents.

Electronic Disclosures and Notices Returned Undeliverable
If your disclosures and notices are returned to us as undeliverable, we will attempt to make contact with you, referencing the phone number or address as shown on our records. If our contact methods prove unsuccessful, we will generate paper disclosures and notices and send to you via the U.S. Postal Service at the mailing address shown on our records, and we will terminate this agreement.

Electronic Address Changes
You agree to provide us with a current electronic address for transmission of your disclosures and notices. You must notify us of any changes to your E-mail address. You may change your email address in-person at any of our branch locations. For security purposes, we may require identification to process your request. We will process your request within 5 business days.

E-Statement Fees and Charges
There are no fees or charges to transmit your disclosures and notices to your electronic address.

Right to Withdraw Consent
You have a right to cancel this Agreement and withdraw your consent to receive your disclosures and notices electronically. You may cancel this Agreement by providing a 30 day written notice by postal delivery addressed to: Queensborough National Bank & Trust Company, Attn: Loan Processing Department, P.O. Box 467, Louisville, GA 30434 or by calling QNBT Loan Processing Department at 478-625-2000 or by emailing QNBT at consumerloanprocessingcenter@qnbtrust.bank . Upon cancellation, QNBT will deliver to you in paper format future disclosures and notices at no charge.
